Ramp Application

Applications may be submitted by an individual or by a referring agency*.

SAWs has two main criteria for determining if the application is a qualified need:

  1. Is this request a permanent need?  (Temporary ramp requests will not be considered.)
  2. Is this request based on financial hardship? (If you are working with an agency* that has determined you have a financial need, their qualification will be considered. Individual applicants must qualify based on that county’s Area Median Income for their county of residence and the total household income of the residents.)

*SAWs defines an AGENCY as any organization that serves you by providing resources that help overcome challenges. Examples of agencies include, home health care workers, social services agencies, township trustees, rehab hospitals, or case managers.
